I have solved this on NBU3.4-5.1 by using lock-files
The first bpstart_notify script makes a policy.started and a
policy.stream.running
the other bpstart_notify scripts makes a policy.stream.running
the bpend_notify scripts removes policy.stream.running and checks if
there is any

drive the tape was mounted on. I think however, that the D:\VERITAS\NetBackup\db\media\errors
file contains the exact information needed.

That's fantastic. So I should probably do frequency based cleaning then
until I go to NBU 6.0? The drives seem to need cleaning after about 500

I have a very similar question. I've got a Solaris 2.8 Master and a
Solaris 2.8 Media Server running 4.5GA. The Master Server lists (in the
GUI) two other Media Servers that no longer exist. How do I remove
them?! Apparently they've been gone for a LONG time, so I'm not as much

Can you do a bprecover to a different server then move the media from there?
Either that, or set the master server up as the alternate restore server for
that media server. It should then be able to do restores, but it might leave
you with some issues with expiring the images. Good luck.
