FWIW, every vault problem I've had from 3.2 on yielded to analysis of
detail.log. There's a a ton of information there.

Doug,
When I've had this problem with my ACS library it turned out to
be a stale MAP file located in /usr/openv/volmgr/misc. Check for a file
called something along the lines of "EJECT*.
