To select something with the 4 gen Apple TV remote, press on the touch pad that
you use to move the cursor around on the screen.

I had this problem too! I had to go to the "language tab" and under that, there
is "english" also "audio described." Or something to that effect. You have to
make sure the language should be "english audio described." Hth

You need to turn on audio description in the Netflix app itself. The
description option in the Apple TV settings is for iTunes. I don't remember
how to turn it on. I do remember it's under language in NetFlix, and you use
