4, which also fitted my 4S, but, I paid for it.
It definitely did not come with any of my iPhones. There's simply no room in
the iPhone box for that thing.

I've had the 3GS, 4, 4S and 5, all from Apple direct, never had a cradle ever.
At this time Apple themselves have not released a cradle, I went to get one
myself on the day of launch.
