It sounds like you had the screen curtain on. Triple tap with three fingers
toggles screen curtain. When screen curtain is on, the screen is blank.

Yes that has happened with my iPhone 4S. Not consistantly, but it has
happened.

This happened with me once before. I turned off voice over then the
pictures were there. Don't know why that was the case or even if that had
anything to do with it.
