I can also confirm this on my iPhone6S. The vibration setting is on in
settings/general/accessibility. When the mute switch on the side of
the phone is set to silent, and VoiceOver is on with speech, the phone
only seems to vibrate once when an incoming call is received and in my

Thinking about it further, when mine doesn't vibrate it doesn't vibrate at
all, not even once. I will need to take more notice in the future, but
thinking about it it seems to only happen for those callers that I have
assigned a custom wring tone too. I don't recall it happening with callers
