Your headphone jack may have a piece of dust blocking it or something of that
sort. Try plugging in and disconnecting your headphones a few times to see if
that helps.

I too am having trouble with voice over. It is sort of hit and miss and I can
usually get it back but it isn’t very reliable. I am trying to get a new phone
but that takes a little time. I wonder if there is a problem with voiceover in
one of the new updates.

I know I posted earlier what I thought was a fix, but it went south on me
again. But I have fixed it now. And OMG, I can't believe it. But it was
dust and gunk in the headphone jack, a whole lot of it!
I got the gunk out using a toothpick and very very gentle scraping action.
