This may be an obvious question - are you booting in OS 8.6 before
attempting to upgrade to OS 9.1 or booting from the 9.1 CD? You won't be
able to run the OS 9.1 installer from OS X and get it to work.
Also, it's a bit late now, but OS X instructs you to upgrade to 9.1 before
installing OS X. Doi
