What I will say might sound (and could be) silly, but I read
somewhere that it will be difficult for an ISPs' mailserver (and
MTA) to talk to an external MTA but only to MUAs (Mail User Agents).
I am just trying to eliminate the possibilities.

if the primary & the backup are on the same subnet.

I've developed sa_filter.pl, a perl-based filter that integrates
with SpamAssassin's spamc.
It's been running in the wild for a few weeks now without any problems,
