shelved image remained when delete shelved instance

Bug description:
Shelve-offload one instance with one attached volume. There was a
bug(https://bugs.launchpad.net/nova/+bug/1406440).
The unshelve op would be failed. Then delete the instance, but shelved image
remaind that should be deleted.

When boot instance without volume, unshelved successfully. The shelved
image deleted when instance delete.
reproduce:
1. boot one instance;
2. create one volume, then attach to the instance;
3. shelve, shelve-offload, unshelve the instance;
4. delete the instance
