I added a second swap device to my OSOL 2009.06 laptop and my system
running Nevada build 124. I can't reproduce this. Both swap devices
appear after reboot.
I would agree with Darren's comments that copies=2 is a better
configuration for a one-disk pool.

Here are some things to try/think about:
1. After a reboot with no swap or dump devices, run this command:
# zfs volinit
Note that volinit is gone in future builds.
