We are about to undergo a penetration test and understand that there is a need to issue a 'Release and Authorization' form. It should state that the testing organization won't be held liable for unintentional interruptions or loss or damage, etc. as a result of the test. What I am looking for is an example of this form, so that I don't have to 'invent' the wording from scratch. Thanking you in advance.
