Covad as others do use a generic password for their routers. If you contact
them, they will usually provide for you.
On other routers, Netopia only offers a reset to default situation.

Subject: Accessing a Netopia/Covad Router
Hello all,
Would anyone be familiar with accessing a Netopia R7200 SDSL Router, for
which the username and password is unknown?
Is there some kind of default password system, maybe something put in by
Covad?
Thanks in advance to anyone with guidance.
With Regards,
Spencer D'oro, MCP
