Hi Yaron,
thanks for the tip. I read the Semantic Forms page several times but i
didn't get it that the page name feature can solve my issue.
I added {{{info|page name=<Person[Given_Name]>_<Person[Name]>}}} to the form
and send the user to Special:AddData/Person. That's it.
