We are facing a similar problem on a different project. Using the http://www.mediawiki.org/wiki/Extension:ArticleComments extension. I'm not sure how its implemented, but the extension skips the capthca when the user submits a comment. I believe there are some 'fixes' for this problem out there, and learning how they work should help you to understand how to add the captcha to the form view of the page. I've been trying to understand at what phase of parsing the captcha comes up, and how to hook into that, but my experience is very limited (sorry about that!).
