If this isn't the restrictive memory setting problem mentioned in the 
INSTALL document's FAQ section, then I don't know.  Do any pages in your 
wiki display?  Did they work before you installed SMW?  What does your 
Special:Version page show?

Turn on debugging (try
http://meta.wikimedia.org/wiki/How_to_debug_MediaWiki ) and you should
see something in the log.

No, that is only if your MediaWiki has additional namespaces besides the 
standard Template:/User:/Help:/Category:, etc.
