At Wake Forest Baptist Medical Center, we have been involved with the early 
identification of sepsis/severe sepsis/septic shock since early 2004. In 2009, 
our Rapid Response Team starting screening their patients for the presence of 
hospital acquired sepsis. We are ready to take the next steps in preventing 
severe sepsis/septic shock by having the bedside nurse screen for sepsis daily. 
The key is prevention. Could you please take a minute and reply to the 
following questions. We are trying to determine the best way to conquer this 
huge project. Thank you for your time.

  1.  Are you using an Early Warning Score to alert your Rapid Response Team? 
If not, what is the trigger to alert your RRT?


  1.  Are you currently screening only one population of patients for the 
presence of sepsis or do you screen all patients?


  1.  What staff members @ your medical center currently screening patients for 
the presence of sepsis/severe sepsis/septic shock [bedside RN, RRT, etc.]?


  1.  How often are patients screened for sepsis [daily, q12 hrs....]?


  1.  Do you use a "Code Sepsis" to alert an interdisciplinary team to the 
bedside for the septic patient? Who is involved on the team?


  1.  What is your inpatient process to assure that the antibiotic for the 
septic patient is administered within 1 hour of identification?


     *   Do you have standing orders for the septic patients antibiotics?
     *   Do you take verbal orders for the antibiotics prior to provider 
arrival?
     *   Do you stock the antibiotics in unit drug dispenser?
     *   Do you have a "Code Sepsis" kit that includes antibiotics?
     *   If so, which antibiotics are available?


  1.  How did you provide sepsis education?
     *   To the bedside nurses?
     *   House staff?
     *   Faculty?
     *   Was the education mandatory?


  1.  What performance feedback do you provide and to whom? And how?
