I'm very curious to understand END time for fluids as well, as this is needed 
to determine persistent hypotension. Must there be documentation from the nurse 
stating an end time, or can we calculate the time of when the fluids were 
finished by how they were ordered.

Example: Fluids ordered at 1000 as "administer 2500 mL bolus over 2 hours". 
Nurse scans the fluids at 1015. Can we use 1215 as the end time, or does the 
nurse have to document this?

Also, what if the order is written as administer a 2500 mL bolus, but with no 
time frame but the term bolus is used in the order?

One of our fallouts has been giving fluids within the 3 hour time frame.  Not 
that the order is not put in, but it is difficult to abstract the completion 
time.  According to one of our CMS webinars, we can base the conclusion of 
infusion of 30 mL/kg of crystalloids from the LIPs order as long as there is a 
duration in the order and a start time by the nurse. (i.e. NS 0.9% 1500cc over 
1 hour).
I am just looking for confirmation that this is an acceptable practice because 
no one else I'm talking too seems to know this.
