Good Morning, In our facility we have a note built into the provider order database that they can use for the focused assessment. This note can be used by itself or added to a previous note or assessment. We also have a quick and very basic note that can be added to the Sepsis Alert note. Our residents respond to the Sepsis Alerts and are responsible for the focused assessments on those alerts and this makes it easy for them to document that assessment and attach it to the alert documentation.
