Good morning, We have a large majority of our severe sepsis/Septic shock patients being discharged to SNF, LTAC, and Hospice. A small portion are discharged home with Home Health. Are there any statistics out there that show where patients are being discharged to? We want to be able to have a benchmark of some sort to go by.
