We had the same issue at my facility, but they are not supposed to code shock 
without clarify (sending a query to the MD). I suggest reviewing the queries 
also b/c sometime not all info is present or they may paint only the picture of 
Septic shock to the provider. I had several cases where is was cardiogenic or 
hypovolemic shock versus septic shock.

Correct, our coders do this.  I believe they can also code septic shock if 
hypotension.

Hello group,

I am being told by our quality department that if a patient is documented with 
severe sepsis and has a lactic acid greater than 4 that it is automatically 
coded as septic shock even though it’s not documented. Is this true?
