Pam; Our workflow is for the RN to inform the provider when IV bolus is complete, take 2 complete sets of vitals at completion time and 15 minutes later, and inform provider of the 2 post-bolus vital signs. The guideline is for the provider to write the sepsis note within one hour after bolus is complete. Admitting providers are educated to place a sepsis reassessment note in their H&P as well - sometimes there are 2 notes and that's OK. We have created smart phrases and notes in EPIC which time stamp the assessment and structure all the needed elements.

So I fail the bundle compliance if there is no documentation found at 3 hr and 6 hr. Even though all other components are present. Most times the patient is leaving the ER before or at 3 hours and by the time the Attending if admitted sees it is outside of 3 hr parameter. I was wondering if anyone else counts the entire bundle compliance as fail if the documentation is not there at the accurate parameter for Focused re-exam. It's frustrating because we don't get credit for what is being done for Bundle compliance for these patients. Would appreciate other observations.
