Molly I would be interested in being included on anything that comes your way on this request. I too believe that our discharge materials for Sepsis are inadequate and would be very interested in finding out what is out there.

Specifically looking for brochures/ teaching materials that include info on post sepsis syndrome as well as addressing risk for developing sepsis again. Also wondering if post sepsis clinics are available in many facilities. If so, how are patients assigned to be seen in the clinic and is there a multidisciplinary team available? Are patients who are discharged to skilled nursing facilities able to benefit from the post sepsis clinics in your hospital systems? Thank you!
