The current patch on antiloop.plone.org is meant for the
LDAPMultiPlugins 1.4 that is in subversion now.  I have a patched
version here for downloading:

I would guess that the failed patching is to blame here.  You could
try an unpatched version of LDAPMultiPlugins and see if that one at
least shows up correctly in that list.  I don't think patching is
absolutely necessary.  An unpatched version should work fine in the
ZMI.  The patch just adds some functions that are really handy to have
in Plone.
