The undo and preferences links have been removed by default. The undo
function actually wasn't all that reliable in a number of use cases and with
Versioning now included by default, it was decided to best to remove the
undo link.
The preferences page is now found via the author page (e.g.
http://plonesite.com/author/LOGINNAME) and clicking on the edit tab or
directly at http://plonesite.com/personalize_form (i think).
To re-enable these links in the personal-toolbar area, go to the ZMI of your
plone site, then:
portal_actions / user and for 'preferences' and 'undo', click on each and
select Visible and save changes.

Do you mean the 'site setup' link in the upper right? Is this with the
default Plone skin or have you installed another theme/skin? Are you sure
you're logging as a user with Manager rights? The zope admin user should do
the trick but it sounds like something has gone amiss with your permissions
and/or skin. Have you changed something in that regard?
As a work around, you should be able to add users with the Managers role by
doing "bin/zopectl adduser" from your zope instance on the file system. Run
that command and it will print some instructions.
