Plone is not "designed" to run behind another system. It's just the most common configuration. I rarely run a production site without at least a caching proxy to speed things up. And while there are some advantages even with a non-caching proxy, the advantages tend to be oversold. You can certainly run a Zope server without a proxy in front if you want.
