Les Hazlewood resolved SHIRO-159.

    Resolution: Fixed

Updated ThreadContext and ThreadState implementations to use remove() in all 
cases.  Everything should be good now (with the same caveat that 
SecurityUtils.getSubject() must still be called downstream from some other 
Shiro component that does the thread binding/cleanup).  All tests, sample apps, 
etc. pass.
