I've applied for an EIN. Its this form: 
http://www.irs.gov/pub/irs-pdf/fss4.pdf. You can't submit it online though. 
So I submitted it via fax a while ago, but haven't heard back from them. I 
will have to chase them up!

Also on a side note, what is a good bank account to use for receiving US 
dollars? I'm using an Australian bank account (Bankwest) and getting stung 
with forex translation fees every time I receive US dollars/US cheques. 
Should I consider getting a US bank account (SVB) or international bank 
like HSBC?
