It is a metal coil embedded in ceramics.  I've had several for several years 
and have not had one break or burn out.  I buy a fixture locally that is a 
metal hood with a crossbar and ceramic socket (a must because of the heat).  I 
try to have the crossbar in touch with my body, with cotton cloth in between, 
no synthetics since they can melt if they should contact the emitter.  It is a 
little tricky figuring out how to hold it where I want it.  I heat the spot for 
as long as I can, results have been even miraculous, as once when I had bad 
neck pains when barely moving my head with numbing sensations, and I lay with 
the emitter over my neck for a couple of hours, and all pain and discomfort 
ceased and didn't return.  I would imagine it was as though the heat relaxed 
the tendons and ligaments and helped them return to a normal position.

The rib took a while to heal, 4 weeks for pain to cease and 8 weeks for a 
pulling sensation to cease.  I had some pain along my vertebrae too, and that 
also ceased, but I've been doing some heavy yard work the last day or so and 
did have some upper back pain recur so used the heat emitter some.
