David,
If you haven't come across it yet you should read rfc2833 RTP payload for DTMF digits.

Subject:        [Sip-implementors] RTP payload type negotiation (DTMF)

Hi All,

Is there an RFC (or draft) available that describes how RTP payload
types are negotiated for DTMF (I'm thinking of the 'telephone-event' SDP
field)?

Any pointers would be appreciated. Thanks,
