My apologies if this has been asked before; I couldn't find it in the 
archives.

RFC 3262 Section 3 says  "If a reliable provisional response is 
retransmitted for 64*T1 seconds without reception of a coresponding PRACK, 
the UAS SHOULD reject the original rquest with a 5xx response."

Which 5xx response is appropriate?

None of the 5xx error responses defined in RFC 3261 seem appropriate:
> 500 Server Internal Error
> 501 Not Implemented
> 502 Bad Gateway
> 503 Service Unavailable
> 504 Server Time-out
> 505 Version Not Supported
> 513 Message Too Large

504 seems like the best candidate, but the description seems vague in 
Section 21.5.5.
> The server did not receive a timely response from an external server
> it accessed in attempting to process the request...
