I can't see any reason for treating NOTIFY from REFER differently from
any other NOTIFY.  E.g., NOTIFY from REFER is within the same dialog as
the REFER, just as NOTIFY from SUBSCRIBE is within the dialog
established by the SUBSCRIBE.

And it's clear that all in-dialog
requests should have the proper Contact value for their end of the
dialog.

Strictly speaking: only in target-refresh or dialog-creating requests or registrations. Contact is not allowed in BYE or CANCEL, but mandatory in SUBSCRIBE and NOTIFY and several others
