In general there is no reason you have to wait till you send the 200.
If the UAC sends an offer in the INVITE, it has to be prepared to
receive media immediately. So the UAS is free to send as soon as it
receives the offer.
And the UAC can start sending as soon as it sees the answer, even if it
gets it in an unreliable 18x.
That of course assumes that preconditions are not in use.
I know there are systems that impose other constraints on this. But in
that case you need to know them - they are proprietary and non-conforming.
