Assuming you mean that the maddr parameter is a parameter on the Contact URI (rather than a header-parameter), it will cause requests from the callee to the caller to go through 10.203.344.46. (Which may not work, since 10.* addresses are private.) ACK, since it goes from caller to callee, will not be affected. BYE will be affected, if it is sent by the callee. The best description I can find is in RFC 3261, section 19.1.1, 3rd paragraph on page 150. You can find the relevant information by searching for "maddr" in RFC 3261.
