I agree with you, addr-spec must be used as well for Refer-To.
However, looks like RFC3515 does not state this explicitly.

For all the headers defined in 3261, which are obtained from ( name-addr
/ addr-spec ), we have a rules saying that the values 

 must be wrapped with '<' and '>', incase addr-spec contains any of
comma, semi-colon or question mark.

 

For example, statement fro 'From' header field:

   "Even if the "display-name" is empty, the "name-addr" form MUST be
used if the "addr-spec"

   contains a comma, question mark, or semicolon.  Syntax issues are
discussed in Section 7.3.1."

 

So, does the value for "Refer-To" also follows same normative rule, as
it's also obtained from name-addr / addr-spec ??

I couldn't find any normative statement in rfc-3515 for this. Thanks!
