AFAIK it is not in any formal spec. It has been discussed on mailing list(s) (ietf lists or this one) from time to time.

This is right on the boundary of what the specs do/don't cover. It is about the UI, not the protocol. You can be conformant to the specs regardless of what you actually render to the user. So it is actually a quality of implementation issue. But it is a problem for those who don't know the implications.
