How would you *not* accept one if it is received? You must do *something*. You *must* minimally wait for a final response, and then do something to terminate the dialog. You might as well try to keep the dialog going.

RFC 6141 section 3.1 discusses a situation potentially involving a user
interaction.

The behavior would depend upon how it needs to behave associated with the
other dialog.  For instance if A actually sent the re-INVITE (such as within
RFC 6141 section 3.1), you can relay the received 18x; however I assume that
isn't the call flow of concern.

In general there are no rules for how B2BUAs behave, other than that they behave properly as a UA independently within each dialog. You need to decide what makes sense in your cases.

Have only seen 200 OK response for a re-INVITE.

Me too (except during testing).

The example in 6141 gives a plausible reason why you might see this, even if it is not commonly implemented. You certainly aren't obligated to create your UAs to initiate this behavior, but you ought to be prepared if you encounter it.
