I'd like to examine this a bit...

Isn't it true that the conference bridge only makes outbound calls when
instructed to do so by some user?  

It seems to me that it would be more appropriate for the bridge to use
the identity of the controlling user when making an outbound call.  That
way, any user could add any destination that that user could call, but
could never get the bridge to make a call that the user could not make
directly.
