It turns out that the machinery in SipRefreshManager was unable to renew a subscription if the initial SUBSCRIBE received a 401/407 authentication challenge. It would resend the initial SUBSCRIBE correctly, but it would never send a refresh SUBSCRIBE. The major visible result was that after about an hour, the RLS ceased receiving status updates from phones, and so the BLF information it distributed stopped being updated.
So BLF has not worked (except shortly after restarting sipX) since whenever in 3.11 we started forcing SUBSCRIBEs to be authenticated, until rev. 14745. If you've been having problems with BLF, please retest with a revision after 14745. If your problem remains, please file an issue.
