Finally I set up an account at Vitelity and within 10
minutes I had a functioning phone system.  I think the
problem may just be the way some of these ITSP provide SIP
services.

Yes, there are many low-quality ITSPs.  Probably the best way to cope with them 
is to find another ITSP.
