Well, the registration should be showing NAT in it and show the remote "internal" ip address of your handset, so it cannot properly "contact" it.
I would make sure the intranet subnets ONLY list the subnets you actually connect to where sipx is located and can reach WITHOUT NAT traversal. I would make sure on both firewalls that your SIP ALG is turned off.
