If there are three phones on three lan's

LAN A
LAN B
LAN C

phone 1 is on LAN A with the SipXecs server
Phone 2 and 3 are on LANs Band C connected to the SipXecs server over the
internet using NAT traversal. Are calls between the phone able to pass RTP
traffic between themselves or does all traffic then have to pass through the
SipXecs server at that point? I ask this because it appears that the SipXecs
server is relaying all RTP traffic itself, and I am curious if it is just a
configuration failure on my behalf.

That is the intended behavior. It's not possible to be more efficient in the
general situation because there are NATs that do not provide truly public
addresses for media streams.
