This does seem to happen with PDFs that have rotated pages. Unfortunately this 
happens when the PDF with embedded notes are saved by Apple's PDFKit code. As 
it's their code, there is nothing we can do about this (except file a bug 
report and waiting 10 years or so). This again shows an important reason why we 
don't save the PDF with embedded notes by default.
