Hi,

If you have Quicksilver, you could create an AppleScript

tell application "Skim"
        tell document 1
                save in path as "PDF With Embedded Notes"
        end tell
end tell

(The document 1 part might need tweaking to make sure it is always the 
frontmost document)

create a trigger in Quicksilver whose sole task is to execute this script, and 
assign it Cmd+S as a hotkey, and limit the trigger’s scope to Skim only. That 
way, whenever you save by hitting Cmd+S in Skim, it will automatically export 
the PDF into a file with the same file name and path and with embedded notes 
(and overwrite the original PDF…).

I am sure similar things would work with other application launchers.
