primary reason for this is the location where you have saved this file. means if you lose network drive and your connection is unstable this can occur.
secondly if you open a file directly from mail client in temp folder then saving after changes gets affected due to any restrictions or bugs in temp folder. hope the above is helpful to solve your problem.
