Answer: don't use wvdial, or get an ISP who doesn't use some silly
        post-dialup menu.

wvdial is an `autodialer' which is designed to be able to handle
*most* [not all] cases by using common replies to common prompts.  It
knows how to deal with some simple menu systems [to which you use
'ppp' as the answer] and how to handle username and login prompts.
Anything more complicated requires editing of the source [its all C++].
