can you reproduce it? It may have been a case where your kid tried to save
something, selected Desktop instead of going into the directory, a dialog
popped up asking if you want to replace the file, which was answered "yes"

That's a wild stab in the dark, but as always steps on how to reproduce bugs
are invaluable for bug reports
