We use it on Rusty (SLUG's mail/web server), and it's the main cause of mailing list outages. That said, we have an extremely manky configuration hacked together over 10 years, so I'm sure a clean room implementation wouldn't have the same issues. >From my experience it does a good job of filtering spam.
