I believe if you attempt to run vmadm update with a add_cdrom.json file specified as shown in the example, it will tell you:
updates to disks are only allowed when state is "stopped", currently: running (running) For me that wasn't a problem because I was able to shut down the VM, make the change and bring it back up again. If you can't do that, one option is to just mount the iso within the KVM guest's OS. On Windows, there are several free utilities to do that. And on Linux, you can just mount the iso using normal mount commands. Others much more knowledgeable than I may have a better idea...
