Well, that's pretty ancient, so it's hard to conclude much from that. zpool_vdev.c actually hasn't changed since 2010, but a lot of other things that could induce this have. I'd suggest looking at the nvlist 'cnv' there. Having ::spa -v or even zpool status might make this clearer also.
